  Important Things to Remember

  • Only products from this Physicians Health Choice catalog are covered under your member benefit.

  • Allowance amounts expire at the end of each quarter (March 31st, June 30th, September 30th and December 31st) and cannot be added to the next quarter.

  • You cannot use future allowance amounts when placing your order. However if you wish to use personal funds for purchases that exceed your allowance, simply provide Fieldtex Products with your credit card information.

  • Please allow 7 - 10 days for delivery of in-stock items. You will be notified if an item is out of stock and may take longer for delivery.

  • If you receive a damaged item, call Fieldtex Products immediately at 1-800-845-0813. The item will be exchanged for an identical item at no cost to you. Please note: Only damaged products can be exchanged within 30 days of purchase, no other returns are allowed.

  • Fieldtex Products is the supplier of all the products in this catalog and can only answer questions about your order. Medical information regarding the use of these products is not available. Please consult your physician.
